Client Rights

  1. Release of Information: You have the right to discuss with your provider what information is in your record, and if you sign a release of information authorizing VCNY to share information with outside sources, you have a right to discuss specifically what information will be released.

  2. You have the right to end your counseling and other services provided by VCNY at any time and request a referral to another service provider.

  3. You have the right to request a different provider to the extent possible if you are dissatisfied with the initial assignment. Reassignment will depend upon availability of alternate staff.

  4. You have the right to be informed about the services available to you with VCNY and how to utilize those services.

  5. You always maintain the right to question the focus of your counseling sessions.

  6. You have the right to present a complaint, knowing that your care will not be compromised in any way.

  7. You have the right to be treated with respect, consideration and dignity without regard to race, national origin, age, gender, sexual orientation, religion, political belief, or handicap.

  8. You have the right to request and receive information concerning your evaluations and other services received in easily understandable terms. This includes your right to receive a summary of your records with VCNY.

  9. You have the right to receive enough information to give informed consent before beginning the counseling process.

  10. You have the right to be informed about your provider’s credentials, record keeping, limitations of services, and to receive a response to any other questions you may have.

  11. You have the right to have your personal health information held in confidence as protected by state and federal law. This information cannot be shared with anyone outside VCNY without your written permission or under circumstances prescribed by law.
